ONLY if you are the official representative of a NEW school applying for the SCHOOL's IHSA membership.
Returning schools need to
login using the school username and password chosen when first applying for membership.
For undergraduate rider membership click on "Individual Rider Application" and for Alumni rider memberships click on "Alumni Application".
The application for new team membership in the IHSA is now available. This form must be filled out by either the team coach or the official school representative. When filling out the form for the first time, you will be asked to choose a username and password FOR THE TEAM. Please make sure that you choose a username that is specific to the school or team and not to an individual as this will be the login for the school from now on. DO NOT choose your usual personal username. The coach will use this username to gain access to specific personal information about team members so it must not be given out to anyone else other than possibly a designated administrative assistant.
Once you have filled out the form, click on the "Continue" button. You will be presented with a list of the information specified on your form to use to make one last check before submitting your 2009-2010 application. This is NOT the formal application form so do not print it. Read over the information one last time. If it is all correct, click on “Submit 2009-2010 Team Application”. If something needs to be fixed, click on “Make Changes”, fix the incorrect data, and click on “Continue” again. Once you are sure that ALL the information is correct and you have clicked on “Submit 2009-2010 Team Application” you will be presented with your filled out 2009-2010 Team Membership Form. Print this form.You then must have the printed form signed by the coach and official university representative, and then send it to your Region President with a school or team check made out to IHSA Inc. for
$250. (personal checks and money orders are not acceptable).
You will receive an email confirmation of your application. If you do not, please email
webcommittee@ihsainc.com.
The Team Membership form must have been completed before any individual riders will be able to fill out their forms.
